Health Assessment Record "Blue Form"
School Physicals are state mandated for newly enrolled students and for students entering kindergarten, grade 6 and grade 10. In addition West Hartford Public Schools requests an updated physical during 3rd grade.  
 
Early Childhood Health Assessment Record "Yellow Form"
 Pre-school children entering school at age 3 should have a physical updated yearly. 

Immunizations Required for School Entry
Up-to-date immunizations are required by the State Department of Education and the State Department of Health. 

Authorization for the Administration of Medication at School
All medications administered at school, must have physician authorization and parent/guardian permission. Self administration of medication may be authorized by the prescriber and parent/guardian and must be approved by the school nurse in accordance with Board policy. In the case of inhalers for asthma and cartridge injectors for medically-diagnosed allergies, students may self-administer medication with only the written authorization of an authorized prescriber and written authorization from a student’s parent or guardian or eligible student.

Food Allergy Management Plan
This document was developed by WHPS to manage food allergies in the school. If your student has a food allergy, please contact your school nurse so we can develop a health care plan that will provide a safe environment for your child.  The plan reflects best practice in managing the school environment, as well as appropriate steps to take should there be an exposure to the food allergen. The health care plan will individualize the plan to the student, based on medical diagnosis, age, and ability. Our goal is to give your student the knowledge and skills to independently manage their health care needs for life.
